Your Treatment (ePRF)
We use a comprehensive ePCR digital platform to help improve data, security, accuracy, and consistency of patient records.
From 1st April 2024, we have moved to a digital platform for reporting the treatment we provide to our patients.
Using mobile tablets, we collect information such as your name, address and the injury you have sustained along with the treatment given. These are then stored on a secure system. Using digital copies also preserves the data and reduces human error.
Just like the paper copies of these forms, you can request a copy for your records.
Getting your Data
If you need a copy of the data we've collected and the treatment you've received you can request that here.
Our team will send this over to you in a PDF format or we can arrange to send a printed copy to your address. A copy of your ID will be required or we will send a link to the phone number given on the form.
